Member pays a procedural fee to the provider on the same day of service. These fees are discounted below usual and customary fees. Please referto your Treatment Plan
for USUAL CUSTOMARY FEES, which are the average cost a patient would expect to pay for a procedure without belonging to Smile Nuvo Dental Benefits Plan. Dental
procedures recommended on your Treatment Plan are discounted 20%, excluding dental products and devices.
